When bleeding is more than just “a bad period”

Heavy periods — also called heavy menstrual bleeding (HMB) or menorrhagia — refer to menstrual bleeding that is heavier, longer, or more disruptive than normal.

Heavy periods are common — but they’re not something you should have to live with.

If your bleeding is affecting your work, sleep, social life, or energy levels, or if you’re needing to change pads or tampons very frequently, it’s worth being checked. Many women are told this is “normal”, but often there is an underlying cause that can be identified and treated.

What causes heavy periods

Heavy bleeding may be caused by:

  • Fibroids — growths in the uterus that can increase bleeding

  • Adenomyosis — when uterine tissue grows into the muscle of the uterus

  • Endometrial polyps — small growths inside the uterus

  • Hormonal imbalance

  • Changes around perimenopause and menopause

  • Rarely, endometrial hyperplasia or endometrial cancer

Finding the cause is important, because treatment depends on what is driving the bleeding.

How heavy periods are assessed

Your appointment will usually include a detailed discussion about your symptoms and cycle, and often pelvic ultrasound.

Advanced ultrasound allows detailed assessment of:

  • The uterus

  • The uterine lining

  • The ovaries

  • Fibroids, adenomyosis, or polyps that may not be found on examination alone

In some cases, a hysteroscopy may also be recommended to look directly inside the uterus.

My training in advanced pelvic ultrasound allows subtle causes of bleeding to be detected and treatment to be targeted rather than based on trial and error.

Treatment options

Treatment is always individual and based on what matters to you. Options may include:

  • Reassurance and monitoring

  • Medication to reduce bleeding

  • Hormonal treatments

  • Hysteroscopy to remove polyps or submucosal fibroids

  • Laparoscopic surgery for conditions such as fibroids or adenomyosis

  • Your bleeding interferes with daily life

  • You have bleeding between periods

  • Your periods are becoming heavier over time

  • You feel tired or have low iron

  • You are trying to conceive
