When cysts cause pain, worry, or affect fertility

Ovarian cysts are fluid-filled sacs that develop on or within the ovaries.

They are very common and often harmless, but some cysts can cause pain, bloating, irregular periods, or fertility concerns.

Many cysts resolve on their own. Others need monitoring or treatment, depending on their type, size, and how they are affecting you.

How ovarian cysts can affect you

Depending on the cyst, you may experience:

Some cysts are found incidentally on ultrasound, even when no symptoms are present.

 How ovarian cysts are assessed

Pelvic ultrasound is the most important tool for assessing ovarian cysts. It allows detailed evaluation of:

  • Size

  • Shape

  • Internal structure

  • Blood flow

  • Features associated with cancer risk

My training in advanced gynaecological ultrasound includes the use of pattern recognition and risk modelling to assess ovarian masses — going far beyond simply measuring a cyst.

In many cases, this allows a cyst to be confidently identified as low risk and safe to monitor, avoiding unnecessary surgery and anxiety.

Routine scans are often unable to provide this level of detail, which can leave women stuck in uncertainty or referred for surgery that may not be needed.

I combine fellowship-trained advanced ultrasound and laparoscopic surgery to assess ovarian cysts carefully and guide the right treatment — whether that means reassurance and monitoring, or surgery when needed.

The goal is always to provide clarity, avoid unnecessary procedures, and protect long-term ovarian and reproductive health.

When surgery is needed

If ultrasound suggests that a cyst is causing symptoms, not resolving, or carries features that need further investigation, surgery may be recommended.

When this is required, I perform minimally invasive (keyhole) surgery to remove cysts while preserving as much healthy ovarian tissue as possible — particularly important when fertility matters.
